I  the parent or guardian, hereby grant permission to Student Achievement Institute (hereinafter referred to as SAI) it's employees, agents, affiliates, and assigns, the right to photograph and/or film continuously and use my child's images and other reproductions of physical likeness for and shall include any still photographs and/or motion picture film or video, theatre, Internet or any other entertainment medium in connection with the exhibition and duplication for the purposes of distribution of said production.

I, further grant permission for the use of my child's images and/or recordings for any advertising, publicity or exploiting of said production. SAI is also granted permission to reproduce any in any manner whatsoever any recordings. I understand these videos collect data to better my player's growth and can be shared and/or published to various media outlets (i.e., social media, YouTube, business promotions, news stations, etc.) for the world to view and share with other individuals.
